Status and influencing factors of self-advocacy in breast cancer patients with chemotherapy

Objective To investigate the level of self-advocacy ability of female breast cancer patients with chemotherapy,and to analyze its influencing factors,so as to provide references for clinical implementation of targeted intervention. Methods A total of 240 breast cancer patients with chemotherapy were investigated by general data questionnaire,Female Self-Advocacy in Cancer Survivorship Scale,Social Support Rating Scale,and Sense of Coherence Scale. Results The self-advocacy score of 240 breast cancer chemotherapy patients was(68.89±10.66);there was a positive correlation between self-advocacy and social support(r=0.723,P<0.001);it was also positively correlated with the sense of coherence(r=0.569,P<0.001). The results of multiple linear regression analysis showed that age,educational level,social support and the sense of coherence were the influential factors of self-advocacy ability of breast cancer patients with chemotherapy,which explained 61.4% of the total variation. Conclusion The self-advocacy ability of breast cancer patients with chemotherapy is at medium level,and there are some problems in self-decision-making,effective communication with medical staff and seeking social support. Medical staff should pay attention to patients with advanced age and low education level,and help patients improve their sense of coherence and their ability to obtain and use social support,so as to improve the level of self-advocacy of patients.

Effect of inspiratory muscle training on clinical outcome of patients with chronic heart failure:an overview of systematic reviews

Objective To evaluate the systematic reviews of the effect of inspiratory muscle training on clinical outcome of patients with chronic heart failure. Methods PubMed,Embase,CINAHL,Cochrane Library,CNKI,Wanfang Database,Chinese Biomedical Literature Database,and VIP Database were searched by computer to find systematic reviews/meta analysis on the effect of inspiratory muscle training on patients with chronic heart failure. The search time limit was from the establishment of the database to January 2023. 2 researchers who were trained in evidence-based knowledge system independently screened the literature and extracted the data. AMSTAR 2 scale was used to evaluate methodological quality,and GRADE system was used to summarize and grade the evidence. Results A total of 14 systematic reviews were included. AMSTAR 2 evaluation results showed that there were 1 paper of high quality and 1 paper of medium quality,and the other 12 papers were of low quality or very low quality. The evidence quality evaluation results of 40 outcome indicators in 14 systematic reviews using GRADE system showed that 3 pieces of evidence were of medium quality,and 31 pieces of evidence were of low quality,and 6 pieces of evidence were of very low quality. The meta-analysis results showed that inspiratory muscle training can improve the maximum inspiratory pressure,6 min walking distance,peak oxygen uptake and dyspnea in patients with chronic heart failure(P<0.05),but the intervention effect on quality of life needs further confirmation. Conclusion Inspiratory muscle training plays a positive role in the clinical outcomes of patients with chronic heart failure. However,considering the overall research quality and the quality of evidence of outcome indicators included in the current systematic reviews are generally low,it is still necessary to further conduct strict,standardized and comprehensive high-quality randomized controlled trials to provide stronger evidence support for the effect of inspiratory muscle training in patients with chronic heart failure in the future.

As a new health care service model,the stepped care model can adjust the intervention strategies in time according to the treatment effect,improve the resource allocation and expand the scope of care services on the one hand,and give personalized care support according to the treatment effect on the other hand,which has the advantages of low cost and high efficiency. In order to provide references for chronic disease prevention and treatment,this article introduces the origin and development,core elements,application and application dilemma of the stepped care model in chronic disease health management.

Construction of a self-management program for diabetic foot amputees

Objective To construct a scientific and standardized self-management program for diabetic foot amputation patients. Methods On the theoretical basis of Bandura theory and Anderson’s behavioral diagnosis model,literature retrieval and semi-structured interview were used to formulate a draft of self-management plan focusing on diabetic foot amputees. From September to November 2022,2 rounds of correspondence were conducted with 31 experts by Delphi method,and indicators at all levels were screened and modified. Results A total of 31 questionnaires were distributed in the 2 rounds of expert consultations,and 31 were recovered. The effective recovery rate of the questionnaires was 100%. The authority coefficient of consulting experts was 0.971;Kendall’s coefficient of harmony was 0.146 and 0.205(P<0.001). In the second round of consultation,the coefficient of variation of experts was 0~0.15,and all items were 0.25. The final scheme included 3 first-level indicators,23 second-level indicators,and 64 third-level indicators. Conclusion The self-management program with diabetic foot amputation patients as the main position is scientific,targeted and feasible,and can provide bases and guidance for diabetic foot amputation patients to carry out effective and standardized self-management.

Video education for advance care planning:a scoping review

Objective To review the application of video education in Advance Care Planning(ACP),and identify the elements of video education for ACP,so as to provide the guidance for future research. Methods A literature search was performed in the PubMed,Embase,CINAHL,Web of Science,PsycINFO,Cochrane Library,CNKI,Wanfang,VIP and CBMdisc. The retrieval time was from the establishment of the database to April 30,2022. The included studies were summarized and analyzed. Results A total of 27 articles were included. Forms of video educational include mobile terminal devices,telemedicine. The core content includes providing ACP related knowledge,providing disease-related knowledge and information on the advantages and disadvantages of treatment measures,clarifying values,goals and medical care preferences. The outcome indicators were mainly the process indicators,action indicators,care outcome indicators,feasibility evaluation. Conclusion Video education in the field of ACP presents preliminary effects and feasibility. It is suggested that researchers could draw on and refer to foreign ACP video education tools,develop and update scientific and feasible ACP video education tools for our country,explore appropriate ACP video education strategies,improve the effectiveness evaluation index,and improve the quality of ACP video education.

Currently,the survival time of colorectal cancer patients has been gradually extended,but cancer and treatment can cause many troubles for them. Patient-reported outcomes with colorectal cancer focus on assessing the physical and psychological experience of patients,which helps to identify their health challenges and give targeted treatment. This article reviews commonly used patient-reported outcome measures in the field of colorectal cancer at home and abroad and their clinical application. The content characteristics,existing problems of the current tools were comparatively analyzed. Future research directions are proposed. It is intended to provide a reference for the application,development,and clinical practice of patient-reported outcome measures for colorectal cancer patients in China.

Nursing management in isolation wards for coronavirus disease 2019 in a general hospital under a joint rescue model

Objective To explore modes and effects of emergency nursing management in isolation wards for coronavirus disease 2019(COVID-19) in a general hospital under a joint rescue model. Methods A joint rescue model was carried out in fever wards at a COVID-19 designated hospital in Wuhan after evaluating the wards environment,medical situation,patients’ conditions and the structure of nursing staff. By restructuring the three-level responsibility system of nursing management,COVID-19 wards were reorganized by rapid screening and isolation of suspected patients; advantageous complementarities in nursing schedule,multi-mode standardized training,double insurance of oxygen supplier for patients during oxygen therapy,seamless supervision on hospital infection prevention,emphasis on physical and mental health of nursing staff were conducted. Results With the close cooperation of medical staff,the joint rescue and support model has been carried out smoothly. 97 cases with COVID-19 were treated in wards from January 27 to March 15th,2020; 84 cases(86.59%) were cured and discharged; 1 patient died(1.03%). There was no nosocomial infection and cross infection among medical staff. Conclusion Amid the COVID-19 epidemic,the number of confirmed cases has risen sharply,while nurses of respiratory and critical care specialty were in shortage in reorganized hospitals. The implementation of the joint rescue model can achieve human resources support and professional technical support,improve the hospital conditions and deficiency of oxygen,increase the cure rate,avoid nosocomial infection,and promote physical and mental health of nursing staff.
